Mechanical malfunction. Structural failure. Human error.

Anti Authority, Impulsivity, Macho, Resignation, and Invulnerability. Poor situational awareness, snap judgments, and lack of a decision making process. Poor risk management and lack of stress management.

Do it quickly to get it over with. It could happen to me. Not so fast, think first.
