Not so fast, think first. Do it quickly to get it over with. It could happen to me.

Risk Management. Safety Management System. Crew Resource Management.

the communications only. all phases of the operation. the flight portion only.

Mechanical malfunction. Structural failure. Human error.

Follow the rules. Rules do not apply in this situation. I know what I am doing.

I can do it. Taking chances is foolish. Nothing will happen.

Anti Authority, Impulsivity, Macho, Resignation, and Invulnerability. Poor situational awareness, snap judgments, and lack of a decision making process. Poor risk management and lack of stress management.

Checklists. Situational awareness. Flying outside the envelope.

Making a rational judgment. Recognizing the invulnerability of the situation. Recognizing hazardous thoughts.

Someone else is responsible. What is the use? I am not helpless.
